بررسی پیامدهای درمان غیرجراحی ترومای غیر نافذ کبد و طحال در بیماران ترومایی مراجعه کننده به بیمارستان امام حسین (ع) تهران

چکیده
زمینه و هدف: مطالعات گذشته نشان داده است که درمان غیرجراحی را می‌توان به عنوان یک رویکرد درمانی در بیماران با ترومای غیرنافذ کبد و طحال در شرایط همودینامیک پایدار در نظر گرفت. لذا این مطالعه با هدف بررسی پیامدهای درمان غیرجراحی در بیماران با ترومای غیرنافذ کبد و طحال مراجعه کننده به بیمارستان امام حسین تهران انجام شده است. مواد و روش‌ها: این مطالعه بصورت مقطعی آینده‌نگر، بر روی 124 بیمار با ترومای غیرنافذ (بلانت) کبد و طحال که طی سال‌های 1399- 1400 به بیمارستان امام حسین تهران مراجعه کرده‌اند، انجام شد. بیماران با شرایط هموداینامیک پایدار و عدم وجود اندیکاسیون قطعی لاپاراتومی اورژانس، تحت درمان غیر جراحی (Conservative) قرار گرفته و در بخش آی سی یو بستری و تحت نظر قرار گرفتند. اطلاعات مربوط به علائم حیاتی و آزمایش خون بیماران شامل هموگلوبین و هماتوکریت در بدو ورود و طی بستری مورد سنجش و ثبت قرار گرفت. همچنین آزمایش پروفایل کامل کبدی، نیاز به دریافت فرآورده‌های خونی، سونوگرافی و سی تی اسکن شکم، و مدت زمان بستری برای تمام بیماران ثبت شد. سپس یک ماه بعد از ترخیص بیماران مجدداً از نظر بروز عوارض احتمالی مورد ارزیابی قرار گرفتند. یافته‌ها: از میان 124 بیمار، 76 نفر مرد (3/61 درصد) که اغلب آنها در رنج سنی 5/4 ± 5/35 قرار داشتند. تروما کبدی در 61 نفر (2/49 درصد)، ترومای طحال در 50 نفر (3/40 درصد) و ترومای همزمان کبد و طحال در 13 نفر (5/10 درصد) از بیماران دیده شد. شکست درمان غیرجراحی در 21 نفر (17 درصد) از بیماران دیده شد. در شکست روش درمان غیرجراحی، سن بالا، درجه بالای آسیب و همچنین وجود ترومای همزمان مغزی در بیماران با ترومای طحال به طور معنادار (P < 0.05) مؤثر بودند. نتیجه‌گیـری: روش درمانی غیرجراحی را می‌توان در بیماران با ترومای بلانت کبد و طحال که شرایط هموداینامیک پایدار و درجه آسیب پایین‌تری دارند، به عنوان یک رویکرد درمانی مناسب در نظر گرفت.
