Article title: Evaluation of Diagnostic Value of Needle Aspiration of Abdomen with Open Peritoneal Lavage in the Patients with Abdominal trauma

Article abstract: Introduction & Objective: Trauma is the most common cause of death in the first three decades of life. An Important way to decrease mortality rate in traumatic patients is the quick diagnosis of organic injuries. Abdominal paracentesis and diagnostic peritoneal lavage are effective techniques for rapid diagnosis of primary injury, before the appearance of clinical signs and symptoms. This study aimed to analyze the diagnostic value of needle aspiration of abdomen with open peritoneal lavage in the patients with abdominal trauma.
Materials & Methods: This study was a randomized clinical trial on 55 patients with abdominal trauma who were admitted to hospital, and had indications of DPL and TAP, without any contraindications of DPL. All patients first tolerated TAP and then DPL and we compared the results through surgical findings. Data from questionnaire was processed with, SPSS version 16, and were presented with tables and descriptive statistics.
Results: Fifty five patients with multiple traumas were studied. Forty seven of them were male with ages between of 70 to 76 years old. In five patients, intra abdominal hemorrhage was reported in the diagnostic TAP, consistent with surgical findings. In 47 patients, TAP was negative, but DPL was positive and consistent with surgical findings. In 2 patients, DPL and TAP were apparently negative, but microscopic examination of DPL was consistent with surgical findings. Sensitivity and specificity of diagnostic TAP in this study were 9.25% and 100% respectively, with negative predictive value of 2%.
Conclusions: In patients with multiple traumas suspected for intra abdominal injuries, diagnostic TAP is not reliable and is not recommended. Open Peritoneal Lavage is a fast, safe, and reliable method for diagnosis of injured viscus in patients with various trauma and abdominal trauma.
